LONGITUDINAL CARE PLANNING

In order to achieve this the providers of care have to have the health information technology infrastructure and the clinical/operational work process to develop, implement, maintain, and measure the clinical outcomes. John has a unique capability of providing value having successfully lead the changing of two LTPAC Corporation clinical technical strategies as well as his dedicated Federal volunteer work to include LTPAC in the total spectrum of care even though the LTPAC sector was not included in the HITEC Act incentive program.
